It is important not to confuse sedation with treating children's teeth under anesthesia. Although it is now possible to administer full anesthesia to a child using inhalation (breathing through a mask), this is the only thing that these two anesthetic procedures have in common. Dental treatment under general anesthesia is rarely used in young children, mainly for complex surgical procedures. It requires special training and supervision by a specialist not only during treatment procedures, but also after «awakening». Sedation is much simpler. Preparation is minimal, and immediately after the end of treatment, the child can return to their usual activities. Sedation has no long-term effects.
In addition, parents note that after dental treatment under sedation, their child goes for preventive check-ups without fear or anxiety.
It follows that sedation forms a positive attitude towards dental treatment in children, which is very important for their entire future life.
Indications and contraindications for sedation dentistry in children
Sedation protects the child's nervous system from unnecessary stress and has no side effects. Therefore, we sincerely believe that all children should have dental treatment under sedation.
The indications for mandatory sedation are:
- the child's fear of the dentist
- the presence of negative experiences with dental treatment
- the child's inability to sit still for long periods of time (hyperactivity)
It is worth highlighting situations in which sedation is not mandatory but highly desirable:
- multiple cavities
- the need to treat several teeth in one visit
- increased gag reflex
Contraindications include:
- American Society for Anesthesia (ASA) class III or IV (severe illness) - when sedation/anesthesia becomes too risky
- the presence of serious respiratory diseases, obstructive sleep apnea, anatomical abnormalities of the respiratory tract - increased risk of complications
- acute infectious diseases, colds, ENT diseases (obstructed nasal/respiratory tract) - inability to adequately control breathing
- Allergies or chronic conditions in which the use of sedatives is either contraindicated or requires special caution
- Behavioral/psychological factors: a child with a pronounced negative reaction, inability to cooperate, when sedation is ineffective without additional behavior modification
Before sedation, the doctor must first conduct a consultation and assess the child's general health.
How is the treatment of baby teeth under sedation performed?
Preparation
Although the treatment of children's teeth under sedation and under general anesthesia are completely different types of anesthetic procedures, they have several things in common:
- the need to conduct tests before starting treatment
- the mandatory presence of an experienced anesthesiologist during the treatment session
In order for sedation to proceed without problems, the child needs to undergo the following tests:
- a complete blood count with leukocyte formula
- coagulogram (for surgical procedures, such as tooth extraction)
- biochemical blood test
- electrocardiogram (for children under 3 years of age, an echocardiogram or ultrasound of the heart is sufficient)
Induction of sedation
Once the anesthesiologist has selected the optimal composition of the sedative mixture, the patient only needs to put on an inhalation mask, take a couple of deep breaths, and that's it. The mind and body are immersed in a state of deep relaxation, fears and anxieties disappear, and the doctor can calmly proceed with treatment.
Coming out of sedation
No special medical procedures are required here. After the treatment is complete, the mask is removed from the child, and within a few minutes, they return to their normal state.
