Methods of therapeutic dental treatment
Caries treatment
This is the most common therapeutic procedure.
Main stages:
- Diagnosis: visual examination, transillumination, X-ray
- Tooth isolation: placement of a rubber dam to protect against saliva
- Removal of affected tissue with a drill
- Filling: application of adhesive and restoration of tooth shape with photopolymer material
- Polishing and bite correction
Modern composites allow for accurate reproduction of the anatomy and color of the tooth.
Pulpitis treatment
Pulpitis is an inflammation of the vascular-nerve bundle inside the tooth (pulp). It requires endodontic intervention.
Treatment methods:
- Vital extirpation: removal of living pulp followed by canal treatment
- Devital extirpation: preliminary administration of a devitalizing agent, followed by pulp removal
- Mechanical and chemical treatment of the canals: using an endomotor and irrigation with antiseptics (hypochlorite, chlorhexidine)
- Root canal filling: filling the canals with gutta-percha and sealant
- Final restoration: restoration of the tooth with a filling or inlay
Treatment of periodontitis
Periodontitis occurs when pulpitis is neglected and affects the tissues around the root of the tooth.
Treatment method:
- Removal of necrotic pulp and rinsing of the canals with antiseptics
- Instrumental treatment of the canals to remove infected dentin
- Medication placement: calcium hydroxide, antiseptics, enzyme preparations
- Filling of the canals after the inflammation has been eliminated
- Follow-up X-ray examination
Sometimes apexification is used in young patients or root tip resection in complex cases.
Direct and indirect tooth restoration
After treating caries or inflammation, it is important to restore the shape and function of the tooth:
- Direct restoration: filling with composite directly in the oral cavity
- Indirect restoration: fabrication of an inlay or onlay in the laboratory and fixation to the tooth (more commonly used in cases of significant destruction)
The choice of method depends on the degree of damage to the crown of the tooth.
Fluoridation and remineralization therapy
Used for caries prevention and treatment of the initial stages of demineralization:
- Professional fluoridation: application of preparations with a high fluoride content (varnishes, gels, trays)
- Remineralization: preparations with calcium, phosphorus, hydroxyapatite
Prescribed as a course (5–10 procedures), especially for children and patients with increased sensitivity.
Installation of posts
If more than 50% of the tooth is destroyed, a glass fiber or metal post can be installed in the root. It serves as a support for subsequent restoration:
- If the post is used to restore the tooth with composite, this is considered therapy
- If a stump inlay is made for a crown, this is an orthopedic procedure
