Teleroentgenography

Teleroentgenography: what it is and why it is needed

Teleroentgenography, or teleroentgenogram (TRG) as it is also known, is a method of X-ray examination of the skull that provides a panoramic image of the head in a lateral or frontal projection, accurately accounting for the distance between the radiation source, the patient, and the image receiver. This ensures that the images have a standardized scale and can be used for accurate measurements.

In dentistry, this type of examination is used to assess symmetry, bite, and the position of the jaws and teeth. The image allows orthodontic treatment and other interventions to be planned with maximum accuracy.

Indications and contraindications for teleradiography

A teleradiograph in lateral or direct projection is prescribed for malocclusion, jaw displacement, preparation for orthognathic surgery, and before orthodontic treatment. Teleradiographs are also taken to monitor the treatment process and evaluate its effectiveness.

Relative contraindications include pregnancy and the inability to hold the head still.

Types of TRG images

How a teleradiograph is performed

Patient preparation:

  • The patient comes to the X-ray room without metal jewelry, earrings, hair clips, glasses — all of these must be removed
  • They are explained the procedure and asked not to move during the scan

Positioning

The patient is placed in a special stand (teleradiograph) that fixes the head in the desired position:

  • Lateral projection (most common) — the left or right side of the head is turned toward the image receiver
  • Frontal projection (face-on) — the face is directed straight at the receiver
  • The head is fixed with the help of a forehead and chin rest or ear clamps
  • The patient is asked to close their teeth in their normal bite position and not to swallow during the scan
  • In some cases, the patient is asked to inhale and hold their breath for a few seconds

Image:

  • The X-ray operator moves behind the protective barrier
  • X-ray radiation is turned on—the exposure lasts less than a second
  • This produces a highly accurate 1:1 image of the skull, which is immediately transferred to a computer for analysis

The entire procedure, including preparation, takes up to 10 minutes.
