In order for an implant to hold a crown securely for decades, it needs sufficient bone in the jaw. But bone is not fat, so the body does not store it "in reserve." And if a single-stage implantation procedure is not performed, the jawbone in the socket area will gradually begin to resorb after tooth extraction. The body no longer needs to support the tissues that "worked" to hold the root in place, so it "breaks them down" for "spare parts." If the tooth extraction was a long time ago, the bone deficiency will most likely be critical. In such cases, to recreate a reliable support for the implant, dental surgeons perform bone augmentation in the jaw.

Indications for bone grafting

Bone grafting prior to implantation is prescribed only for patients who, following clinical examination and CT scan, are diagnosed with critical bone deficiency in the jaw.

Main indications:

  • insufficient height or width of the alveolar ridge
  • severe atrophy after long-term tooth extraction
  • consequences of inflammatory processes (cysts, granulomas), trauma, or complex extractions
  • the need to place an implant in an area where the bone is thin or has defects
  • preparation for implantation in the lateral sections of the upper jaw, especially when close to the maxillary sinus.

It is important to understand that bone grafting during implantation is aimed at improving the functionality of the jaw, rather than improving its aesthetics, and is fundamentally different from gum grafting.

How bone augmentation is performed during dental implantation

Before the operation, CT planning is carried out: the doctor assesses the extent of the defect, the condition of the soft tissues, and selects the restoration technique. Then the intervention itself is performed. In dental surgery, several techniques are used to build up jawbone tissue. At Club 32, the intervention protocol is selected individually for each patient.

The most common method is guided bone regeneration. The surgeon makes a small incision, carefully forms access to the defect area, and prepares the site. Next, bone graft material is placed in the area of bone deficiency, which serves as the basis for the formation of new tissue. A special membrane is placed on top: it fixes the material and prevents soft tissues from growing into the regeneration area. After that, the tissues are sutured. In the postoperative period, the bone graft material is gradually replaced by the patient's bone tissue. This stage takes several months — the exact timing depends on the extent of the intervention and the individual characteristics of the body.

If the bone deficiency is severe, a more complex technique may be used — bone block grafting. In this case, a dense bone fragment is used, which is fixed in the defect area. This type of plastic surgery helps to restore bone volume in two directions at once — in height and width — and prepare the jaw for precise implantation.

The bone block can be obtained in different ways. The most reliable option is considered to be the use of the patient's own bone (autograft). It is usually taken from an area where there is sufficient tissue volume, such as the chin or the angle of the lower jaw, which allows for a strong bone fragment to be obtained and reduces the risk of rejection. Depending on the clinical situation, the doctor may also use donor material (allograft) or synthetic and combined bone grafting materials. The choice depends on the size of the defect, the patient's anatomy, and the objectives of the future implantation.

After the block has healed, the next stage is implant placement.

In some clinical cases, simultaneous treatment is possible: the implant is installed during the same visit, and bone material is added around it to strengthen the support. This option is not suitable for everyone — it is only possible with sufficient primary stability of the implant and strictly defined indications.

After bone grafting, the patient receives recommendations for care, diet, and exercise. To reduce risks, anti-inflammatory drugs are prescribed, as well as follow-up examinations. It is important to understand that strict adherence to the doctor's recommendations directly affects the quality of new bone formation and the final result of implantation.
