Zirconium dioxide (ZrO₂) is a new generation of ceramic, originally developed in the 1980s for joint prosthetics. The material was introduced into dentistry in the late 1990s. Zirconium crowns became widespread in the 2000s thanks to CAD/CAM technologies (computer-aided design and manufacturing of dental restorations). Robotic production made it possible to manufacture dental crowns that are resistant to stress and abrasion with high precision. This material is chemically inert, has low thermal conductivity, excellent biocompatibility, and high mechanical strength (comparable to high-quality steel). Today, zirconium dioxide crowns are used both in single restorations and for bridge prostheses, including implant prostheses. Thanks to their light transmission and the possibility of layer-by-layer ceramic veneering, they provide high aesthetics and invisibility in the oral cavity. Zirconium does not cause allergies, does not react with saliva, and does not change color over time, making it the preferred material for prosthetics for both the front and side teeth.

Indications and contraindications for zirconium crown installation

The installation of zirconium crowns is indicated in the following cases:

• destruction of the crown part of the tooth;
• the need to restore front or side teeth while maintaining perfect aesthetics;
• allergy to metal;
• desire for a long-lasting result with a perfect appearance

Contraindications include:

• uncontrolled bruxism;
• severe inflammatory gum disease;
• significant destruction of the supporting tooth without the possibility of restoration (in this case, implant placement is indicated first, followed by crown placement);
• children and adolescents

Stages of zirconium crown installation:

• Initial consultation and diagnosis. Includes a clinical examination, orthopantomogram (panoramic X-ray), and, if necessary, CT (cone beam computed tomography) and intraoral 3D scanning. This allows for an accurate assessment of the condition of the hard tissues of the tooth, its position in the dental arch, the degree of wear, and the condition of the periodontium. At this stage, the exact cost of treatment is determined
• Tooth preparation. Grinding is performed under a microscope, observing the exact thickness for zirconium — from 1 to 1.5 mm. If necessary, sanitation is carried out: caries treatment, replacement of old fillings, endodontic treatment of canals with sealed obturation
• Taking impressions. There are two approaches: traditional — using silicone impression materials, or digital — using an intraoral scanner with subsequent transfer of the 3D model to the dental laboratory
• Crown fabrication using CAD/CAM equipment. Modeling is performed with an accuracy of 20–50 microns. The zirconium dioxide blank is milled on a high-precision machine, then sintered (fired) at a temperature of about 1500 °C, after which a porcelain veneer is applied or polishing is performed
• Fitting and installation of the zirconium crown. The bite, contact, tightness, and color match are checked. Fixation is performed using glass ionomer or composite cement
• A follow-up appointment in 10–14 days is a mandatory step to assess your bite, gum condition, and adaptation

Caring for zirconia crowns

This type of crown is highly durable (up to 1200 MPa) and bioinert, but proper hygiene and preventive care are necessary to preserve their aesthetic and functional properties:

• Oral hygiene. It is recommended to brush your teeth twice a day with a soft or medium-hard toothbrush, using toothpaste without abrasive particles (RDA <70). This prevents microdamage to the polished zirconium surface and reduces the risk of gum recession at the edge of the crown
• Interdental cleaning. Dental floss and/or interdental brushes of the appropriate size are mandatory. This is critical for treating the area between the crown and adjacent teeth, as plaque retention in this area increases the risk of marginal gingivitis and secondary caries of the supporting tooth
• Irrigator. Using an irrigator with a water pulsation of 1200–1500 pulses per minute allows you to effectively remove microbial biofilm, especially in the gum line area, without damaging the tissue. The water temperature should be room temperature or slightly warm
• Dietary restrictions. It is not recommended to bite nuts, seeds in their shells, ice, and hard candies, as point loads can lead to microcracks in the ceramic veneer or cement detachment. It is also undesirable to open packages with your teeth
• Professional hygiene. Once every 6 months, professional cleaning is performed at the dentist using ultrasound and the Air Flow system with a soft powder (glycine, erythritol). This allows you to remove pigmentation without damaging the surface of the crown and reduces the risk of peri-implantitis and gingivitis
• Regular check-ups. Visits to the dentist every 6-12 months allow you to identify micro-leaks of cement, the initial stages of inflammation, or malocclusion, which can imperceptibly damage both the structure itself and the periodontal tissues

Zirconia crowns are durable but require systematic care, especially in the cervical area. Following these recommendations ensures a service life of 15+ years without loss of aesthetics and functionality.
