Atypical wisdom tooth removal

When a wisdom tooth is positioned at an angle, has not erupted, has a complex root structure, or other complications, its removal requires particular skill on the part of the dentist and accurate diagnosis. Due to its complexity, the procedure is usually longer and requires a longer recovery period. The key to success and your safety is the reliability and experience of the surgeon and examination with modern equipment — we provide all of this in our dental clinic. Atypical wisdom tooth removal — with Club 32, it's painless and safe

An atypical, or impacted, wisdom tooth is the eighth tooth that remains completely or partially inside the gum or bone tissue and cannot erupt on its own. This situation is common and can cause many problems:

  • chronic inflammation
  • displacement of adjacent teeth
  • cyst formation
  • other complications

The removal of such a wisdom tooth is commonly referred to as atypical or complex removal, regardless of whether it is located in the upper or lower jaw. This is actually a rather complex dental operation that requires a highly qualified doctor and precise planning using modern diagnostics.

The surgeons at Club 32, a family dental clinic in the Holosiivskyi district of Kyiv, have extensive experience in performing atypical wisdom tooth extractions on patients of all ages.

If necessary, you can have all procedures related to atypical wisdom tooth removal performed right here at our clinic at a reasonable price.

For those patients who are very anxious or simply afraid of such procedures, we have a special offer – removal under sedation. Although this increases the cost of treatment, it allows for complex wisdom tooth removal not only without pain, but also without stress.

We always make every effort to solve your problem and maintain your long-term health.

In addition, you can pay for the removal of an atypical wisdom tooth in installments and without overpaying.

We do everything to make your treatment effective, affordable, and comfortable.

Indications for complex wisdom tooth removal

The main indication for complex wisdom tooth removal is the presence of pain, inflammation, cysts, or damage to adjacent teeth.

However, in some cases, we recommend that patients have their wisdom teeth removed before they start causing problems. The fact is that unerupted wisdom teeth can negatively affect your bite and gum health. The abnormal position of such teeth in the gums often leads to the development of pericoronitis, osteomyelitis, or periodontitis.

Removal of an unerupted wisdom tooth is also required in cases where the tooth is at an incorrect angle, rests against a neighboring tooth, or grows toward the cheek. If the tooth is completely hidden under the gum and surrounded by tissue, it is highly likely to cause the formation of a cyst and, in some cases, a tumor. Removal of a wisdom tooth inside the gum is also indicated for chronic inflammation, which can be hidden and only detected on an X-ray. One of the common causes of such situations is a lack of space in the jaw, which makes it physically impossible for the tooth to erupt.

Another indication is pressure on adjacent roots, which can cause their resorption (destruction). Such removal is often recommended before orthodontic treatment to ensure the correct position of the teeth in the dental arch.

Preparing for a complex wisdom tooth extraction

Before removing an atypical wisdom tooth, it is important to undergo comprehensive preparation to reduce the risk of complications and ensure a quick recovery.

It all starts with a consultation with a dental surgeon. The doctor examines the tooth, assesses its position, and evaluates the condition of the mucous membrane and bite. X-ray diagnostics (targeted image, orthopantomogram, or CT scan) are mandatory. These studies are necessary to accurately determine the depth of the roots, their shape, and their proximity to nerve channels and sinuses. Based on this data, the surgeon plans the operation.

The patient should inform the doctor about all chronic diseases, allergies, and medications they are taking. If necessary, blood tests and additional examinations are prescribed. A few days before the procedure, it is recommended to take care of yourself to minimize the risk of colds or exacerbations of chronic diseases. We also strongly recommend that you refrain from alcohol and smoking during the preparation period.

On the day of the operation, it is advisable to eat 1-2 hours before the visit, as eating will be difficult after the removal. Women are advised to plan the procedure outside of their menstrual period to reduce the risk of bleeding. Before starting, the surgeon will use anesthesia, and in cases of increased anxiety, may offer sedation. The Club 32 team strongly recommends removal under sedation, but the choice is always up to the patient. Proper preparation facilitates the removal process and minimizes discomfort in the postoperative period.

Atypical wisdom tooth removal procedure

Removing an atypical wisdom tooth is a surgical procedure that requires a high level of professionalism on the part of the doctor. Unlike standard removal, in this case, the dental surgeon cannot simply grab the tooth with forceps and extract it. The roots of wisdom teeth are often curved, and atypical teeth themselves are often partially or completely hidden under the gums or are positioned at an angle. In addition, wisdom teeth are located close to important anatomical structures. Therefore, insufficient precision on the part of the surgeon or medical errors in the preparation for surgery can lead to serious complications. For example, damage to the inferior alveolar nerve, which manifests itself as temporary or permanent numbness of the lower lip and chin. Therefore, at Club 32, the removal of complex wisdom teeth is performed only by surgeons with extensive experience in such procedures.

The removal procedure begins with thorough local anesthesia. If necessary, the removal is performed under sedation, which ensures not only that the patient feels no pain, but also that they are completely relaxed and stress-free. Next, the surgeon makes an incision in the mucous membrane and peels back the gum flap to access the tooth and surrounding bone. If the tooth is deeply embedded, it may be necessary to remove part of the bone tissue using a drill or piezoelectric surgical instrument.

Then the wisdom tooth is carefully divided into fragments: the upper part (crown) is separated from the roots. This step-by-step removal method minimizes tissue damage and reduces stress on adjacent teeth. The separate parts are removed using elevators and forceps. After all the fragments have been removed, the socket is cleaned of small fragments of bone or enamel and treated with an antiseptic.

In the final stage, the surgeon sutures the wound with absorbable sutures and gives recommendations for care. The entire process takes 30 to 60 minutes, depending on the complexity of the clinical situation. Competent performance of the operation by our surgeons avoids complications and ensures rapid healing.

Recommendations after complex wisdom tooth removal

During the first 24 hours after surgery, it is important to apply cold to the cheek to reduce swelling and avoid hot food. It is not recommended to rinse your mouth, drink through a straw, or touch the wound with your tongue. Your diet should be soft and gentle. Smoking and alcohol slow down healing, so it is best to avoid them. Maintain good oral hygiene, but be careful around the stitches. If you experience pain, take the medication prescribed by your doctor and be sure to come in for follow-up appointments.
