1. General Provisions
1.1. This Privacy Policy (hereinafter - the "Policy") has been developed in accordance with the Law of Ukraine "On Personal Data Protection" and other regulatory legal acts. It defines the procedure for the processing and protection of personal data of patients of the Club 32 dental network.
1.2. The purpose of this Policy is to ensure transparency of data processing procedures and protection of patients' rights.
2. Definition of Terms
Personal data - any information about a patient that makes it possible to identify them (full name, date of birth, contact details, medical data, etc.).
Personal Data Controller - a person who determines the purpose of personal data processing.
Personal Data Processor - a person who, on behalf of the Controller, carries out the technical processing of data.
Data processing - any actions involving personal data (collection, storage, use, transfer, destruction).
3. Personal Data Controller
3.1. The joint Controllers of patients' personal data are the business entities that are part of the Club 32 dental network and directly provide medical services to the patient:

3.2. By signing the "Consent Questionnaire", the patient gives consent to the processing of their data by all the above-mentioned entities for the purpose of providing comprehensive medical care.
4. Purpose and Grounds for Personal Data Processing
4.1. We process Your personal data exclusively for the following purposes:
* Establishing a medical diagnosis, preparing and implementing the Treatment Plan;
* Maintaining medical records in accordance with legal requirements;
* Ensuring communication with You (appointment reminders, informing You about results);
* Internal quality control of medical services;
* Carrying out financial settlements for services provided.
4.2. The basis for data processing is Your voluntary and informed consent granted by signing the "Primary Patient Consent Questionnaire".
5. Procedure for Processing and Engagement of Third Parties (Processors)
5.1. To ensure high-quality and modern maintenance of medical records, the Data Controller uses specialized software systems and engages third parties - Personal Data Processors.
5.2. The main Processor carrying out the technical processing and storage of medical data in electronic form (CRM system) is LIMITED LIABILITY COMPANY "CLINICCARDS" (legal address: office No. 139, 4-A Zoolohichna Street, Kyiv, 04119).
5.3. The relationship between the Controller (PE "Stomatolohichna Klinika Klub 32") and the Processor (LLC "CLINICCARDS") is governed by an agreement imposing obligations on the Processor to ensure technical security, confidentiality, and integrity of the data.
5.4. Your personal data may also be transferred to other third parties (dental laboratories, insurance companies, other medical institutions for consultation) only to the extent necessary to fulfill the purpose of treatment and subject to confidentiality requirements.
6. Protection of Personal Data
6.1. We take all necessary organizational and technical measures to protect Your data against unauthorized access, alteration, destruction, or disclosure. Only authorized employees who have undertaken non-disclosure obligations have access to Your data.
7. Patient Rights
7.1. As a personal data subject, You have the right to:
* Know the sources of collection and the location of Your data;
* Аccess Your personal data;
* Request correction of inaccurate data;
* Withdraw Your consent to the processing of personal data by submitting a written request.
8. Data Retention Period
8.1. Your personal data shall be stored for the period necessary to achieve the purpose of their processing, as well as for the periods established by law for storing medical records.
